World Health Organization recommends task sharing (TS) for point-of-care testing (POCT) with lay health workers (LHW) to improve access when professional capacity is limited. Despite many benefits of POCT, TS remains underutilized. This study examined uptake of TS for POCT in national policies and the implementation landscape in 19 African countries from November 2024 to March 2025. A mixed-methods approach included an online cross-sectional survey with stakeholders (national ministries of health, medical associations, private laboratories, implementation supporting partners, LHWs); review of national health strategic and policy documents and key informant interviews (KII) with national laboratory directors. Standardized emails recruited participants, whilst documents came from requests and online searches. Quantitative data were analysed descriptively, and KII data with framework analysis. Of 217 policy documents collected, 197 (91%) were relevant. Over half of national health strategic plans (10/19; 53%) recognize LHWs as vital for expansion of primary healthcare services, but fewer (7/19; 37%) mention TS. While 58% (11/19) of national laboratory strategic plans aimed to expand POCT access and quality, 84% did not mention LHW to support TS. Among national HIV/AIDS strategic plans, 53% (9/17) referenced TS for POCT, mainly for HIV diagnosis; with only one addressing POCT for advanced HIV disease. Outside HIV and malaria, LHW POCT was rarely emphasised in disease-specific strategic plans. Seventy-five stakeholders (67% male) completed the online survey, and six KII were conducted. All reported that LHW conduct POCT, mainly with donor-support. HIV rapid testing was cited as having the most structured training program. National laboratory leaders acknowledged implementation challenges but saw opportunities to expand LHW-led POCT. Shifting from fragmented, disease-specific approaches to multi-disease TS model is crucial for sustainable POCT. Coherent policy and implementation reforms are needed to institutionalize TS amid declining resources. National laboratory leadership should drive the adoption of training and quality assurance for TS for multi-disease POCT.

Zusammenfassung Ziel dieser Arbeit ist es, die ergonomische Belastung des OP-Teams bei offenen, laparoskopischen sowie roboterassistierten Operationsverfahren (RC) zu untersuchen. Mitarbeiter der Salzburger Landeskliniken (SALK) wurden im Dezember 2023 zu einer standardisierten Onlinebefragung eingeladen. Die Ausprägung und die Unterschiede der psychophysischen Belastung für die einzelnen Berufsgruppen und Fachgebiete zwischen den unterschiedlichen Operationsverfahren wurde untersucht. Es nahmen 64/111 (57,7%) Mitarbeiter an der Studie teil, davon waren 30 (46,9%) männlich und 34 (53,1%) weiblich. Die Tätigkeit im OP ist für die Mehrheit der Befragten körperlich belastend, 60,9% der Befragten mussten bereits ärztliche Hilfe in Anspruch nehmen oder ein Schmerzmittel einnehmen. Die am häufigsten betroffen Körperregionen sind Rücken und Nacken, und 17,2% der Befragten halten es für wahrscheinlich, vorzeitig in den Ruhestand gehen zu müssen. Die Mehrheit empfindet Spaß an der Arbeit im OP. Für 61,9% wird die RC als am wenigsten körperlich belastend empfunden. Signifikante Unterschiede in psychophysischen Belastungen und bei Präferenzen für Operationsarten zeigten sich bei Geschlecht, Altersgruppen, Berufsgruppen, Fachgebieten, Arbeitsjahren im OP und biometrischen Daten. Diese Ergebnisse könnten dazu dienen, gezielt gesundheits- und mitarbeiterzufriedenheitsfördernde Maßnahmen einzuleiten und entsprechende Schulungen durchzuführen.

ABSTRACT Background Prospective real‐world data concerning the early and sustained effects of benralizumab on asthma control in patients with severe eosinophilic asthma ( SEA ) is lacking. Methods XALOC ‐2 is a prospective, observational, multi‐national, real‐world study in adults with SEA treated with benralizumab. This integrated analysis assessed Asthma Control Questionnaire ( ACQ ) scores, achievement of 3‐component clinical remission (which included well‐controlled symptoms [ ACQ score ≤ 0.75], no exacerbations, and no use of maintenance oral corticosteroids [ mOCS ]), and other clinical outcomes, over a 12‐month baseline period and up to Week 56. Associations between remission status and key baseline characteristics were also assessed. Results 535 patients were included. Median (interquartile range) ACQ score at baseline was 3.0 (2.2–3.8). At Week 1, 58.0% (282/486) of patients had ACQ score reductions of ≥ 0.5 points (minimal clinically important difference [ MCID ]) and 35.0% (170/486) had reductions of ≥ 1 point (2× MCID ). By Week 56, these increased to 78.6% (276/351) and 62.1% (218/351), respectively. Improved asthma control after benralizumab initiation was similar irrespective of previous biologic use status. By Week 56, clinical remission criteria were achieved in 26.7% (70/262) of patients versus 0% (0/374) at baseline. No mOCS use, lower body mass index, better asthma symptom control and higher peak blood eosinophil count at baseline were associated with meeting 3‐component clinical remission criteria at Week 56. Conclusions Real‐world patients receiving benralizumab showed early and sustained improvements in asthma symptoms, regardless of previous biologic use. More than a quarter of patients achieved clinical asthma remission after 1 year of benralizumab treatment.
