Summary: Pure platelet-derived growth factor BB (PDGF-BB) enhances healing in multiple tissues and is the most thoroughly studied growth factor in medicine. Beyond the thousands of nonclinical studies that define its mechanism of action in tissue repair, more than 140 published human clinical studies have examined the safety and effectiveness of PDGF-BB therapeutics. Manufactured pure PDGF-BB mimics the biologic activity of the endogenous protein and is the active ingredient in 4 US Food and Drug Administration (FDA)–approved products used for the regeneration of skin, bone, mucosa, and gingiva, a diverse array of tissues important to reconstructive surgeons. The recent introduction of an FDA-registered product for use in medical aesthetics demonstrates interest in using this beneficial growth factor more broadly for skin rejuvenation and scar reduction. The authors review the history of clinical development of recombinant pure PDGF-BB, summarize the evidence from more than 30 years of translational research on the applications of PDGF-BB, and propose areas of unmet needs where pure PDGF-BB could potentially enhance patient outcomes. For this review, a literature search was conducted searching PubMed, Web of Science, Google, and clinicaltrials.gov. Evidence-based data from phase 1, 2, 3, and 4 FDA trials including more than 15,000 patients were extracted from systematic reviews and additional primary studies not referenced in systematic reviews. This summary of evidence supports the safety and efficacy of pure PDGF-BB therapeutics in wound healing and tissue repair, can help guide health care providers on the beneficial use of PDGF-BB applications, and provides a framework for future studies to further investigate PDGF-BB in regenerative medicine.

Background: Hemi-hamate osteochondral grafting is a surgical technique that is utilized to reconstruct the proximal interphalangeal (PIP) joint in cases of unstable dorsal fracture-dislocation with >50% articular surface involvement. However, hemi-hamate osteochondral grafting can be technically challenging, has been reported to have various technical modifications, and can lead to complications such as overstuffing of the joint. This surgical technique article describes successful PIP joint reconstruction with use of a hemi-capitate osteochondral graft, which may offer a viable alternative to hemi-hamate osteochondral graft. Description: A volar approach to the PIP joint is utilized, and a trapezoidal incision is made. The skin, subcutaneous tissue, and neurovascular bundles are carefully retracted. The flexor tendon sheath is opened, and adhesions are released to expose the volar plate. The volar plate and collateral ligaments are reflected. The finger is hyperextended to expose the joint. Damaged cartilage and bone fragments are removed, and healthy cartilage is preserved. The capitate-3rd and -4th metacarpal joints are localized with use of fluoroscopy. A transverse incision is made over the joints, and the extensor retinaculum is incised. The capitate is exposed, and the required graft is marked and predrilled. An osteotomy is performed to harvest the graft. The capitate graft is trimmed and placed into the defect in the middle phalanx base. The graft is temporarily pinned with use of a Kirschner wire and then is secured with 2 bicortical screws. The middle phalanx is reduced, and free movement is confirmed. The volar plate is sutured to the collateral ligament. The flexor tendon sheath is passed beneath the flexor tendons to allow free gliding movements over the newly laid graft. The skin is sutured. Alternatives: Hemi-hamate osteochondral graft is the most common alternative to hemi-capitate osteochondral graft. Rationale: The capitate bone may offer several advantages over the hamate bone for PIP joint reconstruction. The capitate has a more uniform articular surface that closely resembles the middle phalanx base, which may reduce the risk of overstuffing1, Additionally, harvesting a graft from the capitate may be less likely to cause donor-site morbidity compared with harvesting from the hamate. Expected Outcomes: Hemi-capitate osteochondral graft is a promising technique for PIP joint reconstruction. In our previous article 1 , the patient achieved good PIP joint movement and stability at the time of the latest follow-up. The osteochondral capitate graft united well, and there were no signs of graft collapse or resorption. The patient had good range of motion and minimal pain, and was able to return to his previous work. Important Tips: Careful attention should be paid to graft size and placement to ensure proper joint congruency and stability.Early postoperative mobilization is important to prevent joint stiffness. Abstract Background Recent advances of graft-versus-host disease prevention strategies have led to improved overall survival after hematopoietic cell transplantation (HCT). Whether cytomegalovirus (CMV) viral load continues to predict CMV disease, overall mortality, and nonrelapse mortality is poorly defined. Methods CMV-seropositive patients undergoing allogeneic HCT between 2007 and 2017 with weekly CMV DNA polymerase chain reaction surveillance and preemptive therapy were analyzed. Multivariate Cox proportional hazards models were used to estimate the association between CMV viral load by day 100 at different thresholds with CMV disease and overall and nonrelapse mortality up to 1 year post-HCT. Results Of 1539 patients who received a transplant in the study period, 1349 survived >100 days after HCT and were included in the analyses. By day 100 post-HCT, CMV reactivation at any level was observed in 76%, with the lowest incidence at all levels in young children. Pediatric patients had significantly less CMV disease compared to the adult population. CMV reactivation was associated with a higher risk of CMV disease (adjusted hazard ratio, 6.8 [95% confidence interval, 3.54–13]); it was also associated with overall and nonrelapse mortality among day 100 survivors, although the association was diminished recently. The strongest correlation was still apparent between viral load and mortality in patients with a viral load >3 log10 and lymphocyte count <300 cells/μL. Conclusions CMV viral load continued to be a strong predictor for CMV disease. With improved transplantation techniques, the association of viral load with overall and nonrelapse mortality is diminished, but the effect was still present in patients with severe immunosuppression.
