    Background: Janus kinase inhibitors (JAKis) represent a new alternative to treat rheumatoid arthritis (RA). The objective of this study was to evaluate the effectiveness, tolerance profile, and maintenance of these treatments (tofacitinib and baricitinib) in real life. Methods: All patients in the rheumatology department of Amiens University Hospital treated by JAKis for RA were included from 1 October 2017 to 20 May 2020. Clinical and biological data were provided retrospectively in this observational and single-center study. We aimed to study the JAKi maintenance rate at 12 months and their clinical and biological safety profiles. Results: Fifty-five patients were included. Drug maintenance at 12 months was 67.6%. Factors associated with poorer maintenance were a higher Charlson comorbidity index (HR 1.311 (1.089–1.579); p = 0.0042), a higher age (HR 1.055 (1.015–1.096); p = 0.0067), and corticosteroids therapy at initiation (HR 2.722 (1.006–7.365); p = 0.0487). The clinical and biological safety profile was generally good. Conclusions: Our study found that a higher Charlson index, age, and corticosteroids appeared to be associated with the earlier discontinuation of treatment. JAKis had a response and tolerance profile in real life at least equivalent to that of biological disease-modifying antirheumatic drugs (bDMARDs).

    QUALITY OF LIFE AFTER BILATERAL ANTERIOR CRUCIATE LIGAMENT RECONSTRUCTIONS

    Introduction. The aim of study was to analyze the influence of bilateral anterior cruciate ligament reconstructions on life quality of patients and their return to sports activities. Material and Methods. Thirty-two operated patients took part in this survey during the period of ten years. There were 5 women and 27 men, their average age being 30.46 years (19-55). The participants answered a modified Knee Injury and Osteoarthritis Outcome Score questionnaire set and gave data about preoperative and postoperative periods. Results. The participants? age and parameters of Lysholm scale did not correlate significantly with the subjective level of physical activity after the second knee surgery. After the first anterior cruciate ligament reconstruction, 4 participants (12.5%) did not return to trainings, while 28 did (87.5%); 8 patients (25%) did not return to competitions and 24 of them (75%) achieved the competition level of sports activities. After the anterior cruciate ligament reconstruction of contralateral knee, 6 (18.8%) did not return to trainings, while 26 (81.3%) did; 15 patients (46.9%) did not return to competitions, while 17 (53.1%) continued to compete without restrictions. The average values of questionnaire scores were between 95.1-98.2 points. Discussion and Conclusion. Resuming the same or higher level of sports activities after the first reconstruction is one of the preconditions for the same injury of another knee. An athlete looses more than two and a half years of competitions on average. Operations of additional meniscus ruptures do not play a crucial role in restitution of sports activities. Although we achieved good operative results, only every second athlete with bilateral injury has returned to sports activities without restrictions after the bilateral anterior cruciate reconstructions.

    Nonasthmatic Churg-Strauss syndrome superimposed on chronic pyelonephritis: a case report

    Churg–Strauss syndrome (CSS) is a granulomatous small-vessel vasculitis. Asthma is seen in the majority of patients with CSS, but atypical nonasthmatic forms of CSS are also being recognized. We herein describe a 67-year-old woman with a history of chronic pyelonephritis and drug allergy reactions who was admitted to our hospital because of worsening renal function preceded by fever, purpura, sinusitis, and a positive urine culture that confirmed a urinary infection. She was initially treated with pipemidic acid for 7 days, followed by clarithromycin for sinusitis. Laboratory tests on admission showed an absolute eosinophil count of 1750 cells/µL and serum creatinine concentration of 4.72 mg/dL. Urine and blood cultures showed no growth. Kidney biopsy revealed crescent formations with diffuse interstitial fibrosis and foci of eosinophil infiltration. An atypical form of CSS was diagnosed based on tissue eosinophilia, peripheral eosinophilia, and sinusitis. Intravenous methylprednisolone and cyclophosphamide pulse therapy together with hemodialysis treatment improved the patient’s clinical condition but did not resolve the kidney damage. The onset of an atypical form of CSS in our patient manifested as symptoms and signs mimicking those of chronic pyelonephritis and drug allergy reactions. The patient’s chronic kidney disease finally progressed to dialysis dependence.
