BackgroundNursing homes in the Caribbean are scarce and the characteristics of their residents have not been previously documented. This study aimed to describe the clinical profiles of residents living in nursing homes in Guadeloupe and Martinique (French West Indies).MethodsThis is a cross-sectional study of the baseline screening data from the KASEHPAD (Karukera Study of Ageing in nursing homes) study. Clinical characteristics and geriatric scale scores, including the Activities of Daily Living (ADL), Mini-Mental State Examination (MMSE), Mini Nutritional Assessment Short-Form (MNA-SF) and Short Physical Performance Battery (SPPB) were collected and analysed.ResultsA total of 332 older adults were recruited between September 2020 and November 2022. The mean age of the residents was 81.3 ± 10.1, with a male–female ratio of 1:1. Diabetes was reported in 28.3% of the residents, hypertension in 66.6% and heart disease in 18.4%. Dementia was diagnosed in 52.3% of the residents and 74.9% had a MMSE score ≤18. The prevalence of Parkinson’s disease was 9.0%. Additionally, 18.4% were unable to perform any basic activities of daily living (ADL score of 0). The prevalence of physical impairment (SPPB < 8) was 90.0%. One-quarter of the residents were classified as undernourished (MNA-SF score ≤ 7).ConclusionResidents in Caribbean nursing homes are younger than in metropolitan France, whereas they present quite similar clinical profiles. Notably, a high prevalence of diabetes, cardiovascular diseases and neurodegenerative diseases was observed. This study represents a preliminary effort to address the knowledge gap regarding the aging trajectories of older adults in the Caribbean and could guide the development of future nursing homes in these countries.

Abstract Background The management of older adults with type 2 diabetes mellitus (T2DM) has not been previously assessed in nursing homes within the Caribbean region. Aims To investigate the prevalence of T2DM among residents of French Caribbean nursing homes and to characterize their health and functional status. Methods This cross-sectional study is based on baseline screening data from the KASEHPAD (Karukera Study of Ageing in Nursing Homes) cohort. Clinical characteristics and standard geriatric parameters were systematically collected and analyzed. Hemoglobin A1c (HbA1c) levels of older adults with diabetes were retrospectively extracted from patient records. Results A total of 332 participants aged 60 years or older were recruited from six nursing homes between September 2020 and November 2022. The prevalence of T2DM among residents was 28.3% (95% CI: 23.5–33.2). Among older adults with T2DM, 85.1% had hypertension, 17.1% had heart failure, and 24.1% had a history of stroke. The mean HbA1c level was 7.32 ± 1.5%. Of the 35 individuals (42.7%) with HbA1c < 7%, 19 (54.3%) were receiving antidiabetic medications. Multivariate analysis identified HbA1c as the sole factor significantly associated with antidiabetic medication use (OR: 1.76, 95% CI: 1.12–3.04). Discussion Older adults with T2DM residing in French Caribbean nursing homes exhibit a high prevalence of cardiovascular risk factors and are at risk of overtreatment. The management of T2DM in this population appears to be driven primarily by blood glucose levels. Conclusion As the prevalence of older adults with T2DM is expected to rise in the Caribbean region, this trend will present significant challenges in delivering tailored care within future nursing home settings.

Introduction: Cognitive impairment is a major cause of dependency in older people. The aim of this study was to identify factors associated with severe cognitive decline, as assessed by the mini-mental state examination (MMSE), in community-dwelling adults aged 55+ in Cameroon. Method: Data are from a cross-sectional survey carried out in Cameroon. The cognitive status was assessed using the MMSE and a score of 18/30 or lower is considered as a proxy of severe cognitive decline. Result: A total of 403 adults participated in the study. Of these, 16 (3.9%) had an MMSE score <18 and were considered to have cognitive impairment. The rate of severe cognitive decline increased with rising age, from 2.1% in those aged 55–64 years, to 3.3% in those aged 65–74, and 11% in those aged 75 and older. The factors associated with cognitive impairment (MMSE score <18) by multivariate analysis in our population are level of education (OR 0.10 [95% CI: 0.02–0.37], p < 0.01), body mass index (BMI) (OR 0.88 [95% CI: 0.78–0.99], p = 0.03), and IADL score (OR 0.12 [95% CI: 0.03–0.38], p < 0.001). Conclusion: The three main factors associated with cognitive impairment were education, IADL (Instrumental Activity of Daily Living) dependency, and BMI. This study shows that among older people in sub-Saharan Africa, the effect of BMI, IADL dependency, and education on cognitive function appears similar to that observed in middle- and high-income countries.
